It's from an electric E-Road scooter, which was sold in the US for a while - but recently seems to have closed up shop. I believe it's a French company (under the name EHM).
This is the bulb from the single headlight. I cannot locate a replacement anywhere - not locally in stores, or anywhere on the Web.
